Predictive value of C-reactive protein-to-albumin ratio for risk of 28-day mortality in patients with severe pneumonia

Abstract Objectives To explore the predictive value of C-reactive protein (CRP)-to-albumin (ALB) ratio (CAR) for risk 28-day mortality in patients with severe pneumonia. Methods A total 152 pneumonia treated from January 2020 to 2022 were enrolled and assigned into survival group (n=107) death (n=45) according their status after treatment 28 d. Their clinical data compared, influencing factors explored by multiple logistic regression analysis. The receiver operating characteristic (ROC) curve was plotted assess CAR predicting risk. prediction model constructed, its efficiency evaluated. Results had significantly older age, Acute Physiology Chronic Health Evaluation (APACHE) II score, Murray Lung Injury Score, Sequential Organ Failure Assessment white blood cell count, neutrophil red volume distribution width, neutrophil-to-lymphocyte (NLR), fibrinogen, procalcitonin, lactic acid (Lac), CRP lower oxygenation index ALB than those (p<0.05). APACHE NLR, Lac independent AUC established 0.826, sensitivity 88.45 % specificity 87.32 %, indicating high discrimination. nomogram when threshold probability 11–93 %.
